Safe healthcare depends on factors spanning the entire healthcare system. We know that adverse events will occur and the reasons are seldom simple. Improving patient safety and learning depends on investigating, understanding, and addressing the complex networks of causal factors at all levels. Norway and England have established national independent investigation bodies to support learning and system improvement. The talk looks into the rationale, expectations and practice in this new area in healthcare.

About the speaker

Siri Wiig PhD, MSc, is Centre Director at SHARE - Centre for Resilience in Healthcare, at the University of Stavanger, Norway. The SHARE centre is the largest research group in Norway doing research on quality and safety in healthcare. Wiig is full Professor of Quality and Safety in Healthcare Systems at the Faculty of Health Sciences, UiS. Key research interests are resilience in healthcare, patient safety, quality improvement, safety investigations, user involvement, risk regulation, leadership, and learning in socio-technical systems. Wiig is Honorary Professor at the Australian Institute of Health Innovation. She has been a member of national public commissions reporting to the Norwegian Ministry of Health and Care Services (2013-2015) and to the Ministry of Defence (2015-2016).
