Description | Innovation in the management of patients with heart conditions20 July 2022A comprehensive and integrated approach to caring for people with common, rare or complex heart conditions delivers better support for patients and their families as well as clinicians. In this webinar, we will speak with clinicians from HeartConnect at Macquarie University Hospital and the Rapid Access Chest Pain Clinics at Royal Hobart Hospital. Innovations providing rapid access to specialist cardiology, whether through a rapid Access Chest Pain Clinic in a hospital or a rapid referral service such as HeartConnect outside of the public hospital system, provide early assessment, diagnosis and treatment and improve outcomes whilst smoothing pathways of care for patients. Such models also support better integration between primary care and specialist cardiology services. Speakers: Associate Professor Yvonne Zurynski leads research into health system sustainability at the Australian Institute of Health Innovation and has a particular focus on improving the integration of care for adults and children. Associate Professor Zurynski is currently evaluating the HeartConnect model. Associate Professor Chrishan Nalliah is an academic cardiologist with an interest in prevention and cardiac arrhythmias. Dr Nalliah is a National Heart Foundation postdoctoral research fellow and is clinical lead for the HeartConnect project. Dr Niamh Chapman is a postdoctoral research fellow at the Menzies Institute for Medical Research and Deputy Chair of the International Society of Hypertension, Women in Hypertension Research Committee and Emerging Leaders Committee of the Australian Cardiovascular Alliance. Dr Andrew Black is a staff specialist at the Royal Hobart Hospital and visiting specialist at Hobart Private. Dr Black has a particular interest in improving systems of care for Tasmanian cardiac patients, and is the clinical lead for the Tasmanian STEMI Network, and Tasmanian principal investigator for the Victorian and Australian Cardiac Outcomes Registries.
