Apply for committee approval

Learn how to apply for Clinical Innovation and Audit Committee (CIAC) approval for your project.

The MQ Health CIAC was established to:

  • provide organisational oversight of clinical innovations, quality assurance activities, clinical audits, case studies and the introduction of new interventions, including devices and surgical procedures
  • ensure the above are reviewed and supported by evidence of safety, effectiveness and financial governance.

Approval and reporting process

Download and complete the following CIAC forms:

Submit them, along with any other necessary supporting documents, to clinical.innovation@mqhealth.org.au.

If your project is one of the above, you will need to apply for approval by CIAC.

CIAC review

Upon receipt of all documents from you, the Clinical Research Support Team will pre-review your application. If any clarifications are required, the team will contact you for further details.

After a review of your application by Clinical Innovation and Audit Commitee, the Clinical Research Support Team will send you approval letter. The approval is valid for 12 months.

You will need to obtain separate financial approval from the Macquarie University Hospital Executive if required.

Timeline: 5–10 working days after complete submission.

Report

You must submit a CIAC report form:

  • on each anniversary of your project approval date, and/or
  • when your project finishes (if it finishes within a year of approval)
